When soreness is worth getting checked
Sudden heat, swelling, or weakness needs prompt care
Fever with a hot, swollen joint needs urgent medical help. Sudden weakness also matters when you can't stand on that leg. After a fall, get help for an oddly shaped or locked joint.
Ordinary wear tends to change more slowly. Redness, drainage, or fever after a procedure isn't routine. Call promptly, and seek urgent help if you feel seriously ill.
The cause needs attention before any planned treatment.
Ongoing limits in your day deserve an exam
Make an appointment when sleep, walking, dressing, or driving keeps getting harder. Swelling that remains, new catching, and lost motion also deserve a check. An X-ray may help when the same joint stays sore.
Take your current pill list and older joint reports. Tell the doctor how long the ache has lasted and which motions hurt. Numbness, weakness, night soreness, and other swollen joints also matter.
You'll usually have the joint moved and pressed in several places. The doctor may test strength and balance as well. Your first appointment should start with the joint, not a treatment pitch.
The exam shows whether a blood-based shot makes sense
Mild wear may leave more choices than severe wear. Major loss of motion may lead to a talk about surgery. Results from a knee study don't automatically apply to your shoulder or hip. Someone else's relief can't predict yours.

A clear choice includes what happens if relief doesn't come
Decide how you'll judge the result before choosing care. Less soreness, better sleep, and longer walks are clear measures. If someone says cartilage regeneration occurred, ask that person for an X-ray or scan from before and after care.
Pain relief and new cartilage aren't the same result.
You'll also need the full cost and any limits during recovery. Ask whether your medicine must change and who handles problems afterward. Blood thinners, bleeding trouble, or a current infection may change the timing.
Waiting or choosing another kind of care is reasonable. A visit may end with exercise, more testing, surgery, or no procedure. Your health and daily needs still guide the choice.

The Cochrane review of exercise for knee osteoarthritis found high-quality evidence that land-based therapeutic exercise provides short-term benefit in pain and physical function, sustained for at least 2-6 months after the programme ends, with mild transient soreness the only reported adverse effect across 45 trials. It is the best-evidenced treatment for this condition and it costs nothing per injection.

FDA states plainly that no stem cell, exosome, stromal vascular fraction, umbilical cord blood, Wharton's jelly or amniotic-fluid product has been approved for the treatment of ANY orthopedic condition - it names osteoarthritis, tendonitis, disc disease, tennis elbow, back pain, hip pain, knee pain, neck pain and shoulder pain individually. The only FDA-approved stem cell products in the United States are cord-blood-derived blood-forming stem cells for disorders of the hematopoietic system, and there are currently no FDA-approved exosome products.

A four-arm, multicentre, single-blind phase 2/3 randomized trial of 480 knee OA patients (KL II-IV) compared autologous bone marrow aspirate concentrate, autologous adipose stromal vascular fraction and allogeneic umbilical-cord-tissue mesenchymal stromal cells against a corticosteroid injection control. At 12 months NONE of the three orthobiologic injections was superior to another, or to the corticosteroid control, and none of the four groups showed a significant change in MRI osteoarthritis score from baseline. No procedure-related serious adverse events occurred.
